Required clinical information — including health history, consent forms, and provider screening — is collected separately through the Hydreight clinical workflow when you proceed toward a visit. Hydreight serves as HeartWell's clinical portal partner; clinical intake, clearance, and treatment documentation occur there, not on this marketing website.

Before services are provided, clients complete required clinical intake, health history, consent forms, and provider screening through that separate clinical workflow. Information you provide through general website forms is kept separate from clinical records maintained in the Hydreight workflow.

Wellness Gatherings

Each guest participating in a HeartWell wellness gathering completes their own required clinical intake and provider screening through the Hydreight clinical workflow before receiving services.
